LASER, IPL & BODY TATTOO REMOVAL CONSENT & TERMS AND CONDITIONS

I hereby authorise Peaches Graham and/or Levi at Just Peachy PMU to perform laser, IPL, and/or body tattoo removal treatments upon myself. I acknowledge that if any unforeseen condition arises during the procedure, the practitioner may use their professional judgement to take any action deemed necessary in the interest of my safety, skin health, and wellbeing.

I understand that laser, IPL, and body tattoo removal are advanced aesthetic procedures designed to remove, reduce, or lighten pigment or tattoo ink. In particular, the use of a Q-switched Nd:YAG laser breaks down carbon pigment and stimulates rejuvenation within the skin. Over time, my body’s immune system will respond, promoting ongoing healing and rejuvenation. Outcomes depend on multiple factors, including my skin type, skin condition, ink type, location, depth, age of tattoo, lifestyle, and individual biological response. I accept responsibility for agreeing to the treatment plan and understand that results vary and cannot be guaranteed.

I understand that multiple treatment sessions are typically required to achieve optimal results. I accept that the number of sessions may vary depending on skin type, tattoo ink, depth, and location. I understand that 100% removal cannot be guaranteed, and some residual pigment or scarring may remain.

I understand that laser and IPL procedures are invasive and may be uncomfortable during and after treatment. I accept that results may vary according to tattoo colour, ink type, or skin characteristics. I acknowledge that temporary side effects are normal and may include redness, swelling, tenderness, blistering, scabbing, peeling, bruising, hyperpigmentation, hypopigmentation, itching, or skin sensitivity. These effects are usually temporary but may vary according to individual response.

I understand that the outcome of treatments may be influenced by the following factors: medication, skin characteristics (dry, oily, sun-damaged), natural skin undertones, alcohol intake, smoking, stress, a compromised immune system, diet, post-procedure care, sun exposure, hormonal cycles, or other individual biological responses.

I understand that strict adherence to pre- and post-treatment instructions is essential. Failure to follow guidance regarding sun protection, skincare, water exposure, physical activity, and other care may result in infection, scarring, pigmentation changes, delayed healing, or compromised results. The clinic cannot be held responsible for outcomes caused by non-compliance.

I understand that a sensitivity or patch test does not guarantee that an allergic or adverse reaction will not occur. I accept full responsibility if a reaction to the procedure, pigment, or anesthetics occurs. I also understand that for tattoo removal, laser may cause temporary frosting of brow hair, which can be tinted once fully healed.

I understand that control or follow-up sessions, which are included in the initial price, must be completed according to my treatment plan. If I do not attend all scheduled sessions, I accept full responsibility for the final result. Follow-up procedures should ideally be performed 4–12 weeks after the initial session to allow the skin to fully heal. Sessions beyond this period may incur an additional fee.

I understand that laser and IPL treatments may not remove all pigment or tattoo ink, and rare adverse outcomes may occur, including burns, scarring, infection, and pigmentary changes.

I confirm that, to the best of my knowledge, I do not have any physical, mental, or medical conditions that could make this treatment unsafe. I understand that the procedure may be contraindicated for clients with certain conditions, including but not limited to pregnancy, active skin infection, uncontrolled diabetes, photosensitive conditions, or use of photosensitive medication. I am at least 18 years old and not under the influence of drugs or alcohol.

IMPORTANT TREATMENT INFORMATION

  • This treatment is a form of laser, IPL, or body tattoo removal.

  • Multiple sessions are usually required; results vary by individual and ink characteristics.

  • Skin reactions such as redness, swelling, blistering, scabbing, peeling, tenderness, or sensitivity are normal and expected.

  • Treatments cannot be repeated on the same area until fully healed, typically 4–8 weeks depending on the procedure and skin type.

  • Sun exposure should be strictly limited before and after treatment to avoid pigmentation changes or burns.

  • Any medications, supplements, or recent procedures that could affect skin response must be disclosed prior to treatment.

  • Pigment may migrate under the skin, and immediate post-treatment colour may appear darker before fading naturally.

I consent to the taking of before and after photographs of the treatment area for documentation, progress tracking, and professional purposes. Images may also be required prior to the appointment for online assessment.

I understand that Just Peachy PMU requires a non-refundable booking fee to secure my appointment. If I cancel or reschedule within five days, the booking fee will be forfeited. Missed appointments are charged 100% of the treatment cost. Additional sessions required due to aftercare, personal preference, or treatment response may incur an additional fee.

I understand that payment is accepted by debit or credit card or cash only, and checks are not accepted. Prices are subject to change.

I acknowledge that the practitioner has the right to refuse, postpone, or discontinue treatment if they believe that proceeding may compromise skin health, safety, or expected outcomes, or if there is misalignment in consultation or undue external influence on my decision. Consultation fees may be retained in such cases.

By signing this form, I confirm that I have read, fully understood, and voluntarily accept these consent and terms and conditions, and that I am undergoing laser, IPL, and/or body tattoo removal treatment of my own free will.
