CONSENT & TERMS AND CONDITIONS

(Including Use of ZO® Skin Health Products)

By proceeding with advanced skin treatments at this clinic, I confirm that I have read, understood, and accepted the following consent and terms and conditions in full.

I understand that advanced skin treatments are performed with the intention of improving the appearance, quality, function, and overall health of the skin. I acknowledge that individual results vary and that no guarantee has been made regarding specific outcomes. I understand that achieving optimal results may require a course of treatments, ongoing professional skincare use, and long-term maintenance.

I confirm that I have already completed all required medical history, consultation forms, and disclosures accurately and truthfully. I understand that failure to disclose complete or accurate information, including medical conditions, medications, recent procedures, allergies, or skin concerns, may increase the risk of adverse reactions, compromise skin health, and negatively affect treatment outcomes. I accept full responsibility for ensuring that all information provided remains current and accurate.

I understand that advanced skin treatments may result in temporary and expected skin responses including redness, swelling, sensitivity, dryness, tightness, peeling, flaking, irritation, inflammation, discomfort, bruising, or breakouts. I acknowledge that treatments performed in conjunction with medical-grade skincare, including ZO® Skin Health products, may intentionally induce visible and sometimes prolonged skin reactions as part of the skin normalisation, correction, stimulation, and renewal process.

I acknowledge that there are inherent risks associated with advanced skin treatments, including but not limited to prolonged irritation, pigmentary changes, infection, delayed healing, textural changes, or scarring, although these outcomes are uncommon. I understand that individual skin response is variable and unpredictable and that the clinic cannot be held responsible for rare or unforeseen reactions where treatments are carried out in accordance with professional standards and established protocols.

I understand that advanced skin treatments may require multiple sessions to achieve optimal results, and that 100 percent success cannot be guaranteed. I understand that failure to attend follow-up sessions or complete the recommended treatment plan may compromise results, and I accept full responsibility for the final outcome should I not adhere to the advised schedule. I acknowledge that any follow-up or review treatments must be carried out within a specified timeframe to be effective and that treatments outside of this timeframe may incur additional charges.

I understand that professional and medical-grade skincare products, including ZO® Skin Health products, may be used during treatment and as part of prescribed homecare. I understand that these products are designed to actively stimulate, normalise, and improve skin function, and that initial reactions such as redness, peeling, dryness, tightness, sensitivity, inflammation, or purging are common and expected. I understand that patch or sensitivity testing does not guarantee that I will not experience an allergic or adverse reaction, and I accept responsibility should such a reaction occur.

I accept full responsibility for using ZO® Skin Health products strictly as prescribed and for following all professional guidance provided. I understand that deviation from prescribed use, combining products with contraindicated skincare or treatments, introducing additional active ingredients without approval, or discontinuing products without guidance may increase the risk of irritation, barrier impairment, delayed healing, or compromised results. The clinic accepts no responsibility for outcomes resulting from misuse of products or failure to follow professional advice.

I understand that strict adherence to pre- and post-treatment advice is essential for skin health, safety, and optimal treatment outcomes. I acknowledge that failure to follow instructions, including sun protection, avoidance of heat, friction, swimming, active skincare, excessive sweating, or other contraindicated activities, may result in complications, delayed healing, pigmentation changes, or suboptimal results. I understand that the clinic cannot be held responsible for outcomes resulting from non-compliance with aftercare guidance.

I acknowledge that treatment results and skin response may be influenced by factors including medication use, skin type, skin condition, lifestyle, sun exposure, immune function, diet, stress, hormonal fluctuations, aftercare compliance, and individual biological response.

I acknowledge that sun protection is a critical component of skin health, particularly when undergoing advanced treatments or using active medical-grade skincare such as ZO® Skin Health products. I understand that inadequate sun protection before or after treatment may significantly increase the risk of pigmentation changes, skin damage, and compromised results.

I understand and accept that the practitioner has a professional duty of care to prioritise skin health and client safety at all times. I acknowledge that the practitioner reserves the absolute right to refuse, postpone, modify, or discontinue treatment at any stage if, in her professional judgement, proceeding may compromise skin integrity, increase risk, or result in harm. This includes, but is not limited to, situations involving impaired skin barrier function, active inflammation, infection, contraindications, inappropriate expectations, or non-compliance with pre-treatment or post-treatment advice. I understand that this decision is made in the interest of skin health and client protection.

I understand that advanced skin treatments may be uncomfortable, and I acknowledge that visible changes such as redness, swelling, bruising, or temporary colour or texture changes may occur immediately following treatment. I understand that the final outcome may take several weeks to become apparent and may vary according to individual skin type, age, condition, or biological response.

I agree to inform any medical or aesthetic professional of recent or ongoing advanced skin treatments if I undergo further procedures, including laser, IPL, or medical imaging where relevant. I understand that my skin may be more sensitive at certain times, including hormonal changes, illness, or stress, and that this may affect treatment comfort and outcome.

I agree to follow all pre-treatment and post-treatment instructions provided by the practitioner. I understand that failure to adhere to these instructions may result in infection, delayed healing, scarring, pigmentation changes, or compromised results, for which the clinic cannot be held responsible.

I understand that I may withdraw consent or discontinue treatment at any time. I acknowledge that any fees paid may be subject to the clinic’s cancellation, amendment, and refund policies. I understand that prices are subject to change, that booking fees are non-refundable, and that additional sessions required due to non-compliance, personal preference, or lifestyle factors may incur additional costs.

I consent to before and after photographs being taken of the treatment area for documentation, progress tracking, and professional purposes.

By proceeding with booking and treatment, I confirm that I have downloaded, read, and fully understood this consent and terms and conditions document, and that I voluntarily agree to treatment under these terms.
